• Dining Club—Just show your card and you’ll get a special, complete meal price at all three Lehigh Valley Hospital cafeterias. Hours are 4:30-6:15 p.m. (dinner every day) and 11 a.m.-1:30 p.m. (lunch on Saturdays and Sundays only). Each meal is $3.65 and includes an appetizer, entrée of the day, side dish and 12 oz. fountain beverage.

• Bonus Travel--Planning an exotic tour? Business trip? Family vacation? Weekend getaway? As a Vitality Plus Gold member, you can make travel reservations conveniently and get cash bonuses through Bonus Travel. You get cash bonuses on airline tickets, lodging, car rentals, cruises and tours. Just make your reservation through the easy-to-use, 24-hour Bonus Travel reservation service. Here’s how to receive your traveler’s savings:
  • Call the travel center toll-free at 1-800-251-2672 and give your member ID number #0688. Both numbers are on your Vitality Plus Gold membership card. To qualify for bonuses you must make reservations through our travel center.
  • Tell the travel agent your destination and the dates you wish to travel. You may book airline tickets, lodging, car rentals, cruises and special tours, both domestic and international. Have your credit card ready if you are purchasing airline tickets.
  • Tickets, itineraries and a cash bonus application will be sent by first-class mail.
  • Save all receipts from your trip.
  • Send itemized receipts to the travel center with your cash bonus application within 90 days of your return. You’ll receive a bonus check within four weeks.
• About Bonus Travel—All travel service is provided by Cendant Travel, Inc., a fully certified, nationwide, independent travel company. The travel center has agents on duty 24 hours a day, seven days a week for your convenience. In addition, for all travel you book through our center, you’re eligible for the following savings and cash bonuses:
  • 5% cash bonus on scheduled airline tickets excluding tax
  • 5% cash bonus on the cost of lodging at every hotel or motel in the United States and many foreign hotels
  • 5% cash bonus on car rentals from licensed car rental companies both here and abroad, including Hertz, Avis, National and Alamo
  • 5% cash bonus on any cruise or tour, including those available through the Vacation Savings Hotline
  • Special hotel discounts available only through Cendant Travel, Inc.
  • Please note that conventions and other special rate groups, government contract rates, and persons who are not members of our program are not eligible for cash bonuses. Consult the travel agent about restrictions. Your hotel bonuses will be calculated on the base room rate excluding taxes, meals and other room charges.
• Vacation Savings Hotline—Vitality Plus Gold members can save 5 to 25% on special vacations and cruises just by calling the Vacation Savings Hotline. To receive your vacation savings:
  • Call the travel center at 1-800-251-2672 and ask for the Vacation Savings Hotline phone number.
  • Call the Hotline to hear a five- to seven-minute recorded description of available trips. Trips change weekly.
  • To book a trip, call the travel center and ask for the Leisure Travel Department. Leisure Travel is open Monday through Friday, 8 a.m. to 6 p.m. and Saturday, 9 a.m. to 5 p.m. (Eastern Time) and is closed Sundays and holidays.
  • Have your credit card handy to pay for your trip. All Vacation Savings Hotline trips must be paid for at the time of booking.
  • Save itemized receipts to claim your cash bonuses.
    
• “Best Fare Search” Guarantee—With the computerized “Best Fare Search” system, Cendant Travel, Inc., gives you the Low Price Guarantee on all domestic airline tickets you purchase through the service. If you find a lower price, call within 30 days of departure with details and proof of availability. Upon verification, we’ll promptly refund the difference. (Prices must be available to the general public at time of booking. Charter, consolidator and group rates are excluded.)
• International Travel, Cruises—If you wish to make reservations for international travel, cruises or tours, ask for the Leisure Travel Department when you call the travel center. When you call, please give the agent your destination and the dates you wish to travel. The Leisure Travel Department is open Monday through Friday, 8 a.m. to 6 p.m. and Saturday, 9 a.m. to 5 p.m. (Eastern Time) and is closed Sundays and holidays.
